This requirement is not met as evidenced by: when requested by the prescribing physician or the Department, arecord of dosages of medications which are centrally stored shall be maintained by the facility. Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above prescribed morning medication administered on 08/10/26 was not initialed/documented on the MARS. Staff conformed the medications were provided to resident but acknowledged that the entry was not completed which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that several window screens had holes, rips, or tears in them which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 08/27/2025 Plan of Correction The facility designated Administrator stated that a review of all window screens will be conducted and any found to have any holes, rips, or tears in them will be repaired/replaced as needed. A statement of correction, along with a copy of the receipt for services rendered, to repair/replace all window screens with holes, rips, or tear will be completed and submitted into CCL by the due date.

(c) Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: (6) Appraisals are conducted on an ongoing basis pursuant to Section 87463, Reappraisals. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in that [1] out of [4] facility resident files did not contain any updated annual medical assessment addressing dementia care changes, if any, which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 08/13/2024 Plan of Correction The facility designated Administrator stated that all residents diagnosed with dementia will be scheduled with their responsible licensed medical professionals to be assessed and have any changes noted and updated on their LIC 602. A statement of correction, along with copies of the updated LIC 602s, will be completed and submitted into CCL by the due date of 08/13/2024.

This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above by the LPA observed the kitchen cabinet latches and locks to be broken, which poses a potential health, safety or personal rights risk to the persons in care.
