Care of Persons with Dementia: (j)The licensee shall have an auditory device or other staff alert feature to monitor exits, if exiting presents a hazard to any resident. This requirement was not met, evidenced by; based on interviews and information obtained R1 is diagnosed with dementia, and eloped from the facility on (3) different occasions. This is an immediate health and safety risk to residents in care.
The Executive Director, will email LPA by 5pm on 05/31/2024, informing LPA that she has contacted there legal team regarding the plan of correction that is required for today's citation and civil penalty. The ED will have (2) weeks from 05/31/2024, to submit the POC to LPA regarding's today's citation.

Executive Director will contact LPA by 5pm on 03/15/2024, to discuss a plan of action on how they will prevent R1 from eloping again and other residents who are diagnosed with dementia and living in assisted living.

Incident Medical/Dental Care. (c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication...facility staff..shall be permitted to assist... (2) Once ordered by the physician the medication is given according to the physician's directions. This requirement was not met, evidenced by, staff did not administer the medication according to doctor's orders. This a health and safety risk to residents in care.
Resident Services Director reported to LPA that a medication in-service started 04/06/2023 and other dates are scheduled for additional training. LPA requested copies of the 1st training that was conducted on 04/06/2023. POC cleared during today's visit.
