Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following:Any incident which threatens the welfare, safety or health of any resident, such as psychological abuse of a resident by staff or other residents, or unexplained absence of any resident. This requirement was not met as evidenced by resident eloping without staff knowledge which posed a potential health & safety risk to residents in care.

This requirement is not met as evidenced by: Deficient Practice Statement Fire Safety - All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. This requirement was not met as evidenced by expired fire extinguisher which poses a potential health & safety risk to residents in care.

POC Due Date: 09/29/2023 Plan of Correction Deficiency corrected during visit. On 09/29/23, Manager on Duty replaced expired fire extinguisher during visit. Copy of purchase receipt for new fire extinguisher was placed at the bottom of new fire extinguisher. Manager on Duty agreed to have fire extinguisher inspected annually for fire safety compliance.
