(h) The following requirements shall apply to medications which are centrally stored: This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, medication audit / review, the licensee did not comply with the section cited above in two (2) out of two (2) residents medications were not properly documented on the centrally stored medications and destruction record with start dates which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 01/22/2026 Plan of Correction Licensee agrees to provide training from an RN to staff who administer medication and conduct a full audit on all medication for both residents to ensure all medications are accounted for and accurately recorded on the centrally stored medication record log.
