Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation the licensee did not comply with the section cited above in one restroom where the hot water measured 99.3 F which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 11/24/2025 Plan of Correction Administrator agrees to adjust the water temperature and submit a proof and a 5 day water temperature to log to LPA by the due date.
87465(h)(6) Incidental Medical and Dental Care. (h)The following requirements shall apply to medications which are centrally stored: (6)The licensee shall be responsible for assuring that a record of centrally stored prescription medications for each resident is maintained for at least one year and includes:(A)The name of the resident for whom prescribed. (B) The name of the prescribing physician. (C)The drug name, strength and quantity. (D)The date filled. (E) The prescription number and the name of the issuing pharmacy.(F) Instructions, if any, regarding control and custody of the medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above as medications for two residents were not documented on the CSMDR and several medications were missing strength information which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 11/24/2025 Plan of Correction Administrator agrees to conduct a staff training on how to correctly document medications on the CSMDR, and will audit the CSMDR for all of the residents to ensure they are accurately filled. Will submit proof of training and letter confirming audit has been completed by due date.

Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in one (1) out of two (2) resident restroons where the hot water measured at 124.2 *F, which poses an immediate health, safety or personal rights risk to persons in care.
POC Due Date: 11/01/2024 Plan of Correction The Administrator agreed to adjust the water temperature and submit proof the temperature is within the required temperature of 105-120 degrees F, by the end of the day.
(2) In addition to paragraph (1), training requirements shall also include an additional 20 hours annually, eight hours of which shall be dementia care training, as required by subdivision (a) of Section 1569.626, and four hours of which shall be specific to postural supports, restricted health conditions, and hospice care, as required by subdivision (a) of Section 1569.696. This training shall be administered on the job, or in a classroom setting, or both, and may include online training. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in two (2) out of five (5) staff which did not have any documented annual required training which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 11/15/2024 Plan of Correction The Administrator agreed they will have both staff obtain their required annual training and submit proof to LPA by 11/15/2024.
(7) Fireplaces and open-faced heaters shall be adequately screened. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in one out of one fireplace which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 11/01/2023 Plan of Correction Administrator will buy a fireplace screen and provide a photo of the screened fireplace to CCL on or before 11/1/2023.

POC Due Date: 11/01/2023 Plan of Correction Administrator will turn down the temperature on the water heaters (facility has two) and send temperature readings to CCL on or before 11/1/2023.
