(B) Bed rails that extend the entire length of the bed are prohibited except for ...hospice care... This requirement is not met as evidenced by: Based on observation and record review the licensee did not comply with the section cited above as R1's bed contained full bed rails and R1 was no longer on hospice which poses a potential personal rights violation to persons in care.
