What Type A and Type B mean
Type A
Violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
Type B
Violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care.

Both classifications are published by California CDSS and are shown as published. SeniorLivingFacts does not rename them or add a severity level of its own.

Repeated topics

Topics cited in more than one report during the last 36 months. A repeat may show a pattern worth asking about. Each date opens its report below.

No topic repeats in the last 36 months

No deficiency topic appears in more than one report during that window. This does not establish that nothing repeated earlier in the five-year record, and it is not a statement about current conditions.

What the official deficiency says

FIRE CLEARANCE - H & S code 1566.45(c) A bedridden person may be admitted to, and remain in, a residential facility that secures and maintains an appropriate fire clearance. A fire clearance shall be issued to a facility in which one or more bedridden persons reside if either of the following conditions are met: (1) The fire safety requirements are met. Clients who are unable to independently transfer to and from bed, but who do not need assistance to turn or reposition in bed, shall be considered non ambulatory for purposes of this paragraph. (2) Alternative methods of protection are approved. This regulation is not met as evidenced by LPA observed on inspection that R1's ambulatory status needs to be updated to bedridden. This is a health and safety risk to resident in care.

Official plan of correction

Administrator to communicate with resident's responsible party and physician for clarification of residents bedridden status. Administrator to submit physician statement if resident is bedridden, and then request for fire clearance to CCL by POC date.

Deadline recorded: Aug 11, 2026. A deadline is not proof that correction was completed.
