POC Due Date: 04/15/2026 Plan of Correction The facility designated Administrator stated that all facility personnel will be updated for annual training topics and hours in order to meet the requirements at this time. A statement of correction, along with copies of all updated training for facility personnel, will be completed and submitted into CCL for review by this LPA by the due date. Updated training will include name of trainer, topics trained and duration, and list of all attendees.

Safeguards for Resident Cash, Personal Property, and Valuables...no licensee or employee of a facility shall: ...accept any general or special power of attorney for any such person. This requirement was not met as evidenced by: Based on the Department's review of documentation appointing the Licensee as R1's POA. This poses an immediate risk to the health, safety and personal rights of residents in care.

What the official deficiency says

87405 Administrator - Qualifications and Duties (a) All facilities shall have a qualified and currently certified administrator. The licensee and the administrator may be one and the same person. The administrator shall have sufficient freedom from other responsibilities and shall be on the premises a sufficient number of hours to permit adequate attention to the management and administration of the facility as specified in this section. When the administrator is not in the facility, there shall be coverage by a designated substitute who shall have qualifications adequate to be responsible and accountable for management and administration of the facility as specified in this section. The Department may require that the administrator devote additional hours in the facility to fulfill his/her responsibilities when the need for such additional hours is substantiated by written documentation. This requirement has not been met as evidenced by: Administrator's certificate expired 03/16/2022 which poses a health, safety or personal rights risk to residents in care.
