What the official deficiency says

87211 Reporting Requirements (a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following: (1) A written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of the occurrence of any of the events specified in (A) through (D) below. This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case. This is not met as evidenced by: The licensee did not ensure that an incident report was sent to the department within 7 days. This poses a potential health, safety, and personal rights risks to persons in care.

The facility will undergo training from a vendorized trainer for no less than (1) hour in duration on the topic of reporting requirements. A statement of correction and acknowledgement along with documented proof of scheduled training topics, dates, and times will be completed and submitted into CCL by the due date.

What the official deficiency says

All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in by not ensuring the the exit gates were unlocked. LPA Pascua observed a padlock on the black gate which prohibited exit access. This poses an immediate health, safety, or personal rights risk to persons in care.

POC Due Date: 08/29/2025 Plan of Correction Administrator took padlock off the black gate during visit. In addition, administrator states that a tour of the facility will tour of the facility every other day to ensure that the locks are not present. A statement of correction will be sent to LPA by POC date.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and interview, the licensee did not comply with the section cited above by not ensuring the medications were not transferred between containers. LPA observed that the medication for noon pass was pre-poured prior to administration, this poses a potential health,safety, and personal rights risks to persons in care.

What the official deficiency says

(a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in by not ensuring the the laundry room and pull cords in the bedrooms were in good repair. LPA observed two large holes, water damage under the water spouts, and baseboards, and floorings throughout the laundry room was lifting off. LPA learned through interview that the pull cords in each resident bedrooms have not been in working condition. This poses an potential health, safety or personal rights risk to persons in care.

POC Due Date: 10/13/2023 Plan of Correction The administrator stated that a plan to repair the laundry room and pull cord system will be conducted and be provided to the LPA by the POC date. In addition, the administrator shall provide receipts and proof of service to the LPA by the POC date.
