(4) The licensee shall assist residents with self-administered medications as needed. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPA, Admin, and licensee observation and record review, the licensee did not comply with the section cited above in he following medications had counts that were off based on the start date and dosage required: Finasteride 5mg date started was 5/20/26. Bubble pack had 31 tabs, one tab per day was supposed to be given but 18 tabs remain. Losartan 50mg date started was 5/12. Bubble pack had 31 tabs but 2 remain. Pantoprazole 20mg was started 5/12/26. Bubble pack had 31 pills but 4 pills remain, which poses an immediate health, safety or personal rights risk to persons in care.

(h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPA, Admin, and licensee observation and record review, the licensee did not comply with the section cited above in that facility is pre-pouring medications, which poses an immediate health, safety or personal rights risk to persons in care.
