This requirement is not met as evidenced by: Based on observation and interviews, the licensee did not comply with the section cited above in that rodents, rodent feces and a rodent trap were observed in the kitchen and kitchen pantry areas which poses a potential health, safety or personal rights risk to persons in care.
Licensee or Administrator shall submit a plan on what changes will be made to the facility’s physical plant and or operational procedures that will ensure that the facility be kept free of rodents, vermin and insects by POC due date of 8/21/2025.
