87455(c)(2) Acceptance and Retention Limitations (c) No resident shall be accepted or retained if any of the following apply: (2) The resident requires 24-hour, skilled nursing or intermediate care as specified in Health and Safety Code Sections 1569.72(a) and (a)(1). This requirement was not met as evidenced by: Based on interviews and document review it was determined that the licensee should not have accepted the resident back in the facility due to health decline after a series of hospital and skilled nursing stays which poses a potential health and safety risk to residents in care.

Administrator agrees to submit a plan to LPA outlining the steps he will take to determine whether a resident is suitable to be accepted back into the facility after discharge from hospital and/or skilled nursing. Administrator to submit plan to LPA by 06/28/2024.

(a) Disinfectants, cleaning solutions, poisons, firearms and other items which could pose a danger if readily available to clients shall be stored where inaccessible to clients. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in one cleaning solution was found under the sink in the kitchen which posed a potential health, safety or personal rights risk to persons in care.

What the official deficiency says

(a) Based on the individual's preadmission appraisal, and subsequent changes to that appraisal, the facility shall provide assistance and care for the resident in those activities of daily living which the resident is unable to do for himself/herself. Postural supports may be used under the following conditions: (3) A written order from a physician indicating the need for the postural support shall be maintained in the resident's record. The licensing agency shall be authorized to require other additional documentation if needed to verify the order. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview with Adminstrator and record review, the licensee did not comply with the section cited above in two of six residents with postural supports do not have a medical order which posed a potential health, safety or personal rights risk to persons in care.
