(h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above by having R1's medication not centally stored and in residents bedroom R1 is not allowed to adminster own medication which poses an immediate health, safety or personal rights risk to persons in care.

(a) Disinfectants, cleaning solutions, poisons, firearms and other items which could pose a danger if readily available to clients shall be stored where inaccessible to clients. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. LPA observed 7 containers of dish washing detergent on the ground. While touring the bathroom #2, which is located across from bedroom #3, LPA observed a bottle of dish washing detergent below the sink. This poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in. Residents R1, R2 and R3 are under the age of 60. The facility has a census of 4 and the facility did not request an exception request for the residents who are under the age of 60 years. This poses/posed a potential health, safety or personal rights risk to persons in care.

What the official deficiency says

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidenced by: Based on the observvation, mold was observed on the ceiling of one of the restroom which posed a immediate health and safety risk to residents in care.

Administrator stated he/she will submit a plan of correction by the POC due date to prevent the similar incident to occur again. LPA checked the bathroom today and there is no mold observed in the bathroom.
