This poses an immideate safety and health risk to residents in care.

Maintenance shall include ... for the safety and well-being of residents, employees and visitors. This requirement was not met as evidenced by; Based on LPA's observation, bedroom #4's screen door has holes and is not in good repair. ADM confirmed that the screen door is damaged as well. This poses/posed a potential health, safety or personal rights risk to persons in care.
