What the official deficiency says

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in one of the facility sliding glass doors, which could not properly be locked or opened, and one out of six resident bedrooms, which had a hole in one of the walls, which poses a safety risk to persons in care.

POC Due Date: 08/23/2025 Plan of Correction Licensee agrees to submit a a Plan of Correction by POC Due Date stating how the licensee will repair the sliding glass door and the hole in one of the resident bedrooms. Once repairs are made, the licensee shall submit photographic evidence of the repairs.
