This requirement was not met as evidenced by: Based on interview and record reviewed, R1 sustained an eye injury on 2/14/2023 or 2/15/2023, R1's eye worsen before the scheduled eye doctor appointment. The facility did not seek for timely medical attention for resident R1 which poses an immediate health, and safety risk to persons in care.

What the official deficiency says

The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided when such observation reveals unmet needs. When changes such as unusual weight gains or losses or deterioration of mental ability or a physical health condition are observed, the licensee shall ensure that such changes are documented and brought to the attention of the resident's physician and the resident's responsible person, if any. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review & interview, the licensee did not comply with the section cited above in 3 Out of 3 resident records reviewed. LPA requested to review R1-R3's weight records. ADM stated she only measures the residents weight when they go to their annual doctors visit. This poses/posed a potential health, safety or personal rights risk to persons in care.
