What the official deficiency says

(a) Based on the individual's preadmission appraisal, and subsequent changes to that appraisal, the facility shall provide assistance and care for the resident in those activities of daily living which the resident is unable to do for himself/herself. Postural supports may be used under the following conditions: (3) A written order from a physician indicating the need for the postural support shall be maintained in the resident's record. The licensing agency shall be authorized to require other additional documentation if needed to verify the order. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review and observation, R3 has a bed rail that extended the entire length of the bed and R3 is not receiving hospice care at this time which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 03/31/2026 Plan of Correction Administrator stated to submit a written plan of action of understanding of the regulation and not use the full bed rail and ask for written physician’s order for full half-bed rail on R3's bed and place it in R3s record by POC due date.

(h) The following requirements shall apply to medications which are centrally stored: (6) The licensee shall be responsible for assuring that a record of centrally stored prescription medications for each resident is maintained for at least one year and includes: (A) The name of the resident for whom prescribed. (B) The name of the prescribing physician. (C) The drug name, strength and quantity. (D) The date filled. (E) The prescription number and the name of the issuing pharmacy.(F) Instructions, if any, regarding control and custody of the medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in 2 out of 6 Centrally Stored Medication and Destruction Records which poses/posed a potential health risk to persons in care. 2 out of 6 Centrally Stored Medication Records were missing a medication.

POC Due Date: 03/12/2025 Plan of Correction Licensee agrees to conduct staff training by POC date on assuring that a record of centrally stored prescription medications for each resident is maintained. Once training is completed, the Licensee agrees to submit training records including names of staff trained, training date(s), and names and qualifications of trainer to CCL.
