REAPPRAISALS The licensee shall request that all residents receive an annual routine visit with a licensed medical professional once every 12 months...Documentation of the annual routine visit, such as a visit summary, shall be added to the resident's record. This requirement is not met, as MD report for client #2 is dated in 2022. Licensee failed to ensure that residents have annual documented MD assessments, which poses a potential health, safety or personal rights risk to residents in care.

REPORTING REQUIRE Notes:///882582B5004E0AE8/StartUp?OpenFrameset San Bruno ASC-14MENTS Licensee shall furnish to CCLD reports, including written report within 7 days of the occurrence of an epidemic outbreak, which threatens the welfare, safety or health of residents, personnel or visitors. Report shall be made within 24 hours either by telephone or fax to CCLD & to the local health officer when appropriate. Report shall include the resident's name, age, sex, date of admission... This requirement was not met, as 12 staff with Covid were not reported to CCLD until LPA called administrator in January 2022, & CCLD was not notified of COVID staff in July 2021, which posed a potential health & safety risk.
