(29) All equipment, fixed or mobile, and dishes, shall be kept clean and maintained in good repair and free of breaks, open seams, cracks or chips. This requirement is not met as evidenced by: Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in LPA observed 3 garbage cans in the kitchen have dried brown, black, white and yellow spots and particles on the outside which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above as LPA observed R5 is on oxygen, but the facility could not prove that a report was made in writing to the local fire jurisdiction that oxygen is in use at the facility. which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 07/22/2026 Plan of Correction The administrator/licensee will develop a plan to ensure compliance and the plan should indicate that the local fire jurisdiction has been notified for all the current residents who are on oxygen. The administrator/licensee will submit a copy of the plan to CCL by 7/22/2026.

87705 Care of Persons with Dementia (d) The licensee shall ensure that the facility has an auditory device or other staff alert feature to monitor exits on exterior doors and... This requirement is not met as evidenced by based on observation, interview and record review, R1 resides in the Memory Care and was escorted to the first floor for a holiday celebration and exit the facility unattended which poses an immediately health and safety risk to residents in care.

Official plan of correction

The administrator/licensee will develop a plan to ensure there is a staff alert feature to monitor exits on exterior doors when the auditor device was not activated and will submit a copy of the plan to CCL by 6/19/2025. The plan shall indicate staff training.
