What the official deficiency says

This requirement is not met as evidenced by:(2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above as LPA observed hot water temperature in the bathroom/shower rooms in the hallway and bathrooms in resident rooms was measured at 123-125 degrees F which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 04/24/2026 Plan of Correction The administrator will develop a plan of correction to ensure hot water temperature is within the range of 105- 120 degrees F and will submit a copy of the plan of correction by 4/24/2026. In addition, the administrator will take hot water temperature at varies locations in the facility for the next 7 days and provide a copy of the results to CCL by 5/4/2026.

What the official deficiency says

87411 Personnel Requirements - General (a)Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs. This requirment is not met as evidenced by while giving medication, S1 did not ensure it was right the medicaiton and the right resident which resulted in R1 who was giving medication that was intended for R2 which poses an immediate health and safety risk to residents in care.

The administrator/licensee shall develop a plan to ensure the facility has a process to monitor the competency of staff members with medication administration after training. The administrator/ licensee will provide a copy of the plan to CCL by 11/14/2025.
