All window screens shall be clean and maintained in good repair. This facility was found to be deficient as evidenced by the lack of window screens in the facility windows which posed a potential risk to the Health, Safety, and Personal Rights to all residents in care.

The following space and safety provisions shall apply to all facilities: The premises shall be maintained in a state of good repair and shall provide a safe and healthful environment. This facility was found to be deficient as evidenced by the observation of the wood flooring in the common areas that was in need of repair/replacement at this time. In addition, the tiles in the facility restrooms were cracked and/or broken in need of repair/replacement at this time which posed a potential threat to the Health, Safety, and Personal Rights of all residents in care.

The facility designated Administrator stated a review of all floor surfaces in this facility will be conducted and any areas that are in need of repair/replacement, will be repaired/replaced as necessary. This requirement is not met as evidenced by: Deficient Practice Statement Based on LPA Jensen's observation of 2 window screens that were torn and 1 screen that was missing the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
