What the official deficiency says

(c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication, but can communicate his/her symptoms clearly, facility staff designated by the licensee shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (2) Once ordered by the physician the medication is given according to the physician's directions. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in that the start dates, documentation, and med counts for the resident medications were not aligned with one another. The start dates did not coincide with the number of medications left from the bubble packs that were already dispensed at this time. This poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 01/31/2026 Plan of Correction The facility designated Administrator stated that all facility staff providing care and supervision to the residents will be trained, for no less than (1) hour in duration, on the topic of proper handling, dispensing, and documentation of the facility resident medications. A statement of correction, along with updated staff training, will be completed and submitted into CCL by the due date for review by this LPA.

(c) The medical assessment shall include, but not be limited to: (5) The determination whether the person is ambulatory or nonambulatory as defined in Section 87101, Definitions, or bedridden as defined in Health and Safety Code section 1569.72. The assessment shall indicate whether nonambulatory status is based upon the resident's physical condition, mental condition, or both. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in [1] out of [5] facility resident files did not have a complete medical assessment missing the ambulatory/non ambulatory status determination which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 02/06/2026 Plan of Correction The facility designated Administrator stated that all resident medical assessments will be audited to make sure that they are current and complete with the ambulatory/non ambulatory status determination. A statement of correction, along with updated LIC 602s, will be completed and submitted into CCL by the due date for review by this LPA.

What the official deficiency says

The licensee shall have and maintain a current, written definitive plan of operation for the facility. The licensee shall operate the facility in accordance with the terms specified in the plan of operation and may be cited for not doing so pursuant to Health and Safety Code section 1569.49. The plan and related materials shall be on file in the facility and shall be submitted to the licensing agency with the license application. Any significant changes in the plan of operation which would affect the services to residents shall be submitted to the licensing agency for approval. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that the garage area was converted into a staff room with (2) beds present to accommodate live-in staff which was originally not fire cleared as a staff room which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 09/04/2025 Plan of Correction The facility Licensee stated that the garage area will be cleared of all beds and items designated for staff use and return it back to the original set up as the facility garage only. A statement of correction, along with a recent photo of the cleared garage area, will be completed and submitted into CCL by the due date.
