REAPPRAISALS The licensee shall request that all residents receive an annual routine visit with a licensed medical professional once every 12 months, either in person or by video appointment. Documentation of the annual routine visit... shall be added to the resident's record. Documentation of a resident's refusal to receive an annual routine visit...shall be added to the resident's record. This requirement is not met, as MD reports for 6 out of 7 clients are dated over 12 months ago, altho MD signed medication lists are dated in 2026. No MD identified for client #3 MD report.

(c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication, but can communicate his/her symptoms clearly, facility staff designated by the licensee shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (2) Once ordered by the physician the medication is given according to the physician's directions.
