What the official deficiency says

(B)Once ordered by the physician the medication is given according to the physician's directions. This requirement has not been met as evidenced by: Based on documentation, the licensee failed to ensure 1 out of 51 residents received the correct prescribed medication. On 02/13/2026 staff gave R1 the wrong medication. This is an immediate health & safety risk to clients in care.

The licensee will provide proof of medication re-training for S1 conducted in February 2026 with sign in sheet and training topic clearly noted to the LPA via email by 03/19/2026.
