87465 Incidental Medical and Dental Care (h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in that resident medicatons were stored unlocked in the facility refrigerator, which poses a potential health and safety risk to 5 of 5 residents in care.

POC Due Date: 07/18/2025 Plan of Correction Administrator will obtain a locked box to store medications in the facility refrigerator. Administrator will provide in-service training for staff on proper medication storage and will submit a copy of the staff sign in sheet to the Department by POC due date of 7/18/2025.

87705 Care of Persons with Dementia (c) Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: (5) Each resident with dementia shall have an annual medical assessment as specified in Section 87458, Medical Assessment, and a reappraisal done at least annually, both of which shall include a reassessment of the resident’s dementia care needs. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in that 3 of 6 residents who were diagnosed with dementia did not have a medical assessment which was done annually which poses a potential health risk to 3 of 6 persons in care.

87468.1 Personal Rights of Residents in All Facilities: “(a) Residents in all residential care facilities for the elderly shall have all of the following personal rights: (1) To be accorded dignity in their personal relationships with staff...” This requirement was not met, as evidenced by: Based on records and interviews, licensee’s staff (S1 and S2) did not treat 1 of 6 residents (R1) with dignity, which posed a potential personal rights risk to persons in care.

Records and interviews showed S1’s employment was terminated on 05/31/2023, and S2’s employment was terminated on 10/05/2022. Licensee agreed to utilize a third-party source to retrain their direct care staff on Resident’s Personal Rights (see Regulation 87468.1 and 87468.2). Licensee agreed to submit the training sign-in sheet to LPA, by the POC due date.
