(a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following: (1) A written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of the occurrence of any of the events specified in (A) through (D) below. This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case. This requirement is not met as evidenced by: Deficient Practice Statement Based on interview, the licensee did not comply with the section cited above in that the Licensee did not submit an incident report for Resident 1's hospitalization which poses a potential safety risk to 2 of 2 residents in care.

POC Due Date: 05/08/2026 Plan of Correction Licensee stated she will fill out and submit an incident report regarding R1's change in condition and hospitalization. Licensee will also ensure staff complete reporting requirement training and will submit a copy of the incident report and proof of training to the Department by POC due date of 5/8/2026.
87465 Incidental Medical and Dental Care (h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that resident medications were stored in an unlocked kitchen refrigerator which poses a potential safety risk to 2 of 2 residents in care.
Official plan of correction
POC Due Date: 06/20/2025 Plan of Correction Administrator relocated the medications to the locked garage refrigerator during the visit and is planning on purchasing a lockbox to store medications in the refrigerator. Administrator will conduct in-service training for caregivers regarding medication storage and provide the sign-in sheet to the Department by POC due date of 6/20/2025.
(e) Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degree C) and not more than 120 degree F (49 degree C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that the water temperature measured at 140.5 degrees F in a bathroom used by residents which poses a potential safety risk to 1 of 1 residents in care.

POC Due Date: 05/13/2024 Plan of Correction Administrator adjusted the hot water heater during LPA's visit. The Administrator will record the water temperature in downstairs bathrooms every other day in a hot water log and submit a copy of the log to Department by POC due date of 5/13/2024.
