What the official deficiency says
(h) The following requirements shall apply to medications which are centrally stored: (2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that resident medications were stored in the unsecured kitchen refrigerator which poses an immediate health and safety risk to 5 of 5 residents in care.
Official plan of correction
POC Due Date: 04/03/2026 Plan of Correction Administrator relocated the medications to the refrigerator in the facility garage during the visit and will be obtaining a lock box to store medications that require refrigeration. Administrator will conduct in-service training with staff regarding medication administration and storage and will provide the Department with a photo of the lockbox and a copy of the staff sign in sheet by POC due date of 4/3/2026.

What the official deficiency says
(2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that the hot water was measured at 130.8 degrees F in a common bathroom, which posed a potential safety risk to 5 of 5 residents in care.
Official plan of correction
POC Due Date: 04/03/2026 Plan of Correction Administrator turned down the hot water heater during the visit. Staff will conduct regular hot water temperature checks and log the reading from 3/24/2026 through 3/30/2026. Administrator will provide a copy of the hot water log to the Department by POC due date of 4/3/2026.

(h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that 5 of 5 residents' daily medications were prepoured into daily pillboxes, which poses a potential health and safety risk to 5 of 5 residents in care.

POC Due Date: 04/03/2026 Plan of Correction Administrator stated that staff will no longer prepour medications. Administrator will conduct in-service training with staff regarding medication administration and storage and will provide the Department with a copy of the staff sign in sheet by POC due date of 4/3/2026.
