Termination of admission agreement upon death...removal of resident’s property; refund of fees paid; notice of contract termination and refund. A residential..shall not require...notice for terminating an admission agreement upon...death...No fees shall accrue once all personal property...is removed. This requirement was not met as evidenced by: Based on interviews and record reviews, the Licensee did not ensure a refund of pre-paid fees after the death of R1. This poses a potential personal rights risk to 1 out of 5 clients in care.

What the official deficiency says

Care of Persons with Dementia. The licensee shall have an auditory device or other staff alert feature to monitor exits, if exiting presents a hazard to any resident. This requirement was not met as evidenced by: Based upon interviews conducted, R1 was able to leave the facility unassisted and without being detected, which posed a potential safety risk to R1, who was 1 of 5 residents in care.

House Manager offered to install auditory devices on all exit doors and provide proof of installation by the POC due date. House Manager also offered to provide in-service training to all staff regarding proper supervision of residents. Proof of training will be provided to Community Care Licensing by the POC due date of 9/24/2021
