What the official deficiency says
87303(a) The facility shall be clean, safe, sanitary and in good repair at all times. Not Met evidenced by the following: LPA observation and photos of missing (plug plate Room#1) or broken plates for light switches in room numbers two (2), four (4), & five (5). LPA observed missing smoke detectors in the hallway between rooms three (3) to six (6) and a missing smoke detector in room five (5).

Licensee will have all repairs by POC date. Facility Staff installed new plug plate, light plates, and smoke detectors at time of visit.

In addition to the rights listed in Section 87468.1, Personal Rights of Residents in All Facilities....residential care facilities for the elderly shall have all of the following personal rights: (1)To have a reasonable level of personal privacy in accommodations.... Not Met evidenced by the following: LPA during interviews of Administrator, two (2) Staff, cell phone video on Staff/Caregivers phones, Resident#2 was videoed by cell phone in Resident#2 bedroom while having multiple behaviors without consent.
Official plan of correction
All staff will stop taking videos immediately of Resident#2 in the bedroom and Licensee will get documented signed consent from power of attorney by POC date. Licensee will have all staff in - serviced with material, sign in sheet on personal rights by POC date.
