(6) When requested by the prescribing physician or the Department, a record of dosages of medications which are centrally stored shall be maintained by the facility. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above tow (2) out of two (2) residents in care by not maintaining a list of all PRN and prescribed medication and destroying medication that is no longer in use which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 11/10/2025 Plan of Correction Licensee agrees to complete a statement of understanding regarding the above cited regulation and complete a training with all staff regarding centrally stored medication. Licensee agree to submit a list of all who participated in the training by POC due date.

(h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview and record review, the licensee did not comply with the section cited above two (2) out of two (2) residents in care by not ensuring that medication is not transferred from it's original container, staff are not initialing the MARs in future dates, and staff are not preparing medication to be dispensed days in advance which poses/posed a potential health, safety or personal rights risk to persons in care.

87705 Care of Persons with Dementia (f) The following shall be stored inaccessible to residents with dementia (1) Knives, matches, firearms, tools and other items that could constitute a danger to the resident(s). Based on observation and interview the licensee did not comply with the section cited above evidenced by having the key to open the knives hanging from the knives drawer that allows the residents access to the knives which poses an immediate health, safety, or personal rights risk to persons in care.

The licensee has agreed to remove the key hanging from the knives drawer and find a secure place to store the key. The licensee has agreed to train the staff on properly securing the knives and send LPA proof the training was completed by POC due date 9/7/2023.
