What the official deficiency says

(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 87411, Personnel Requirements - General. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and record review, the licensee did not comply with the section cited above in 2 out of 3 staff files by not ensuring that the personnel file maintained a health screening and TB test results. The personnel record were missing the two (2) health screening report and one (1) TB test results which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 06/26/2026 Plan of Correction Administrator agrees to review all personnel files to ensure all file are complete and maintained. Regarding staff one (S1) the health screening and TB test will be completed upon S1 return. S2 will complete the health screening within the next 30 days and notify LPA by POC due date.

87458 Medical Assessment - (a) Prior to a person's acceptance as a resident, the licensee shall obtain and keep on file, documentation of a medical assessment, signed by a physician, made within the last year. This requirement was not met as evidenced by: Based on observation and interviews, the licensee did not ensure resident in care had all documents as regulated which poses an potential Health and Safety risk to persons in care.
