What the official deficiency says

(h) The licensee shall request that all residents receive an annual routine visit with a licensed medical professional once every twelve months, either in person or by video appointment. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, record review, the licensee did not comply with the section cited above two (2) out of six (6) residents in care by not ensuring each resident had a current physician report completed and in their files which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 10/27/2025 Plan of Correction Licensee agrees to review each residents file for a current physician report, schedule an appointment if needed, and ensure physician report is in each residents file for review. Licensee will submit a statement of understanding for the regulation cited above and proof of current physician reports for the two residents by POC date.
