Removal of records shall be subject to the requirements..This requirement is not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above by not by the requested documents to CCLD. LPA made several request via telephone call and text messages which poses a potential health, safety or personal rights risk to persons in care.

(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above by not ensuring that all disinfectant, and cleaning solutions were secured and inaccessible to residents in care which poses a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and record review, the licensee did not comply with the section cited above in two (2) out of three (3) residents in care by not maintaining the centrally stored medication log with the residents MARs to ensure that all medication dispensed was logged which poses a potential health, safety or personal rights risk to persons in care.
