What the official deficiency says

(2) Eight hours of in-service training per year on the subject of serving residents with dementia. " This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and record review, the licensee did not not comply with the section cited above by not ensuring that 3 out 3 staff members did not have annual RCFE training. This poses a potential health, safety and personal rights risks to persons in care.

POC Due Date: 07/31/2025 Plan of Correction Licensee shall conduct RCFE training for all staff. A copy of staff training shall be send to the LPA by POC date.

What the official deficiency says

87203 Fire Safety All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above by not ensuring the the fire extinguishers have been serviced within the last 12 months. LPA observed the fire extinguisher was last serviced in November 2023. This poses an immediate health, safety, and personal rights risks to persons in care.

POC Due Date: 07/18/2025 Plan of Correction During the course of this visit, LPA observed the local fire extinguisher company arrive at the time of LPAs visit. Citation will be cleared today.
