(d) If the resident is unable to determine his/her own need for a prescription or nonprescription PRN medication, and is unable to communicate his/her symptoms clearly, facility staff designated by the licensee, shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (2) The date and time of each contact with the physician, and the physician's directions, shall be documented and maintained in the resident's facility record. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, facility's MAR records were missing signatures/initilas for residents, R1,R2's medications for FEB.2024 which poses an immediate health, safety or personal rights risk to persons in care.

87355 Criminal Record Clearance (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1569.17(b) shall prior to working, residing or volunteering in a licensed facility: (2) Request a transfer of a criminal record clearance as specified in Section 87355(c). This requirement is not met as evidenced by: Based on observation and interviews, S1 and S2 have been working in the facility for without having their criminal record clearance transfered. Per Guardian, S1 and S2 are not associated to the facility. This poses an immediate health and safety risk to resident's in care.
