What the official deficiency says
(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by: Deficient Practice Statement LPA observed that facility's staff left cabinet open in main hallway which has disinfectants, cleaning solutions and gardening tools were accessible in backyard area which were accessible to residents and poses a immediate health and safety risks to residents in care.

(c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication, but can communicate his/her symptoms clearly, facility staff designated by the licensee shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (2) Once ordered by the physician the medication is given according to the physician's directions. This requirement is not met as evidenced by: Deficient Practice Statement Based on staff interviews, observation, record review, facility did not comply with Regulation as medications administration records were not matching for 2 (R2, R4) out of 4 residents, MAR records were not found for February 2026, expired medication for resident R2, which poses an immediate health, safety or personal rights risk to persons in care.

POC Due Date: 02/03/2026 Plan of Correction Administrator shall send letter of understanding of this Regulation and shall conduct staff training for medications management and send records to department by 02/03/26.
