Administrator agrees to submit a statement of understanding regarding fingerprint clearance needed before staff are able to begin working at the facility. All POC documents are due by 01/29/26. Immediate civil penalty of $1000.00 were assessed today due to staff, S1 and S2 were working at facility without fingerprint clearance.

Each personnel record shall contain the following information: This requirement is not met as evidenced by: Deficient Practice Statement Based on staff interviews and record review, it was observed that 3 staff files were incomplete ( No physical, TB Test, LIC508 ) and/or not available for department review upon request which poses/posed a potential health, safety or personal rights risk to persons in care.
