Administrator agrees to submit a statement of understanding regarding fingerprint clearance needed before staff are able to begin working at the facility. All POC documents are due by 01/29/26. Immediate civil penalty of $1000.00 were assessed today due to staff, S1 and S2 were working at facility without fingerprint clearance.

(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: This requirement is not met as evidenced by: Deficient Practice Statement Based on staff interviews and record review, it was observed that 3 staff files were incomplete ( No physical, TB Test, LIC508 ) and/or not available for department review upon request which poses/posed a potential health, safety or personal rights risk to persons in care.
