What the official deficiency says
(c) Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: (5) Each resident with dementia shall have an annual medical assessment as specified in Section 87458, Medical Assessment, and a reappraisal done at least annually, both of which shall include a reassessment of the resident’s dementia care needs. This requirement is not met as evidenced by: Deficient Practice Statement Based on interview and record review, the licensee did not comply with the section cited above with R1 whose LIC602 was last updated 9/10/2021 when R1 has a diagnoses of dementia which poses a potential health, safety or personal rights risk to persons in care.

What the official deficiency says
(a) Prior to, or within two weeks of the resident’s admission, the licensee shall arrange a meeting with the resident, the resident’s representative, if any, appropriate facility staff, and a representative of the resident’s home health agency, if any, and any other appropriate parties, to prepare a written record of the care the resident will receive in the facility, and the resident’s preferences regarding the services provided at the facility. (1) At a minimum the written record shall include the date of the meeting, name of individuals who participated and their relationship to the resident, and the agreed-upon services to be provided to the resident. This requirement is not met as evidenced by: Deficient Practice Statement Based on interview and record review, the licensee did not comply with the section cited above with R2 who did not have a current record of care on file and did not have one to show the licensing agency during the visit which poses a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation Fire Safety: All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. During a facility inspection on 4/14/22, LPA Delgado observed a master bedroom hall closet being used as staff bedroom. This poses a potential health and safety risk or personal rights risk to persons in care.
