Topics cited in more than one report during the last 36 months. A repeat may show a pattern worth asking about. Each date opens its report below.

No topic repeats in the last 36 months

No deficiency topic appears in more than one report during that window. This does not establish that nothing repeated earlier in the five-year record, and it is not a statement about current conditions.

What the official deficiency says

RESIDENT RECORDS: (a) The licensee shall ensure that a separate, complete, and current record is maintained for each resident in the facility or in a central administrative location readily available to facility staff and to licensing agency staff. This requirement was not being met as evidenced by: During the visit, LPA requested to review R1's file, a copy of the LIC500 and Resident roster. Staff could not provide the requested documents during the time of the visit. This poses an immediate health and safety risk to residents in care.

Official plan of correction

Licensee will email all copies of R1's file, LIC500 and Resident roster to LPA by POC due date.

Deadline recorded: Mar 18, 2025. A deadline is not proof that correction was completed.
