(a) The licensee shall have and maintain a current, written definitive plan of operation for the facility. The licensee shall operate the facility in accordance with the terms specified in the plan of operation and may be cited for not doing so pursuant to Health and Safety Code section 1569.49. The plan and related materials shall be on file in the facility and shall be submitted to the licensing agency with the license application. Any significant changes in the plan of operation which would affect the services to residents shall be submitted to the licensing agency for approval. The plan and related materials shall contain the following: This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, and record review, the licensee did not comply with the section cited above by not reviewing, updating, and signing the Infection Control Plan, and Emergency Disaster Plan annually which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 01/29/2026 Plan of Correction Licensee agrees to review, update, and sign the above mentioned plans annually. Licensee will complete a statement of understanding by POC due and submit to LPA.
(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening as specified in Section 87411, Personnel Requirements - General. This requirement is not met as evidenced by: Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above 1 out of 2 staff by not ensuring the staff health screening and TB test result were able for review and in the personnel file posed a potential health, safety or personal rights risk to persons in care.
POC Due Date: 01/22/2026 Plan of Correction Licensee agrees to review and update all staff files, submit a statement of understanding regarding the above regulation, and provide proof of the health screening by POC due date.
(4) The licensee shall assist residents with self-administered medications as needed. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and record review, the licensee did not comply with the section cited above in 1 out of 2 resident in care by not ensuring the medication was properly dispensed. LPA observed the medication was logged as issued, but was still in the bubble pack which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 02/15/2026 Plan of Correction Licensee will complete a training with staff regarding medication error, the MARs, Centrally stored medication. Submit a statement of understanding regarding the regulation.
(c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication, but can communicate his/her symptoms clearly, facility staff designated by the licensee shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (3) A record of each dose is maintained in the resident's record. The record shall include the date and time the PRN medication was taken, the dosage taken, and the resident's response. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and record review, the licensee did not comply with the section cited above by not ensuring the Centrally stored medication log is maintained current dates, destruction log, and monthly changes to medication that are destroyed which poses a potential health, safety or personal rights risk to persons in care.

(e) Water supplies and plumbing fixtures shall be maintained as follows: (3) Taps delivering water at 125 degree F (52 degrees C) or above shall be prominently identified by warning signs. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, and interview, the licensee did not comply with the section cited above in 2 out of 3 area in the home by not ensuring the tap water measure within regulations. The reguired temperature is 105-120 degrees F which poses an immediate health, safety or personal rights risk to persons in care.
POC Due Date: 04/17/2025 Plan of Correction The Administrator agrees to post signs stating " Caution Hot Water " , create a system to monitor the water temperature, and the adminstrator immediately adjusted the water heater. The adminstrator agrees to complete all the POC by due date.
(f) All personnel records shall be available to the licensing agency to inspect, audit, and copy upon demand during normal business hours. Records may be removed if necessary for copying. Removal of records shall be subject to the following requirements: This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 staff by not ensuring the staff personnel records are available for review by CCLD or any other authorized agency, LPA was not able to verify health screening for staff or any pertinent information which poses an immediate health, safety or personal rights risk to persons in care.
POC Due Date: 04/11/2025 Plan of Correction Administrator agrees to have all staff personnel records available for review at the facility for CCLD and other authorized agency by POC date.
(6) When requested by the prescribing physician or the Department, a record of dosages of medications which are centrally stored shall be maintained by the facility. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 residents MARs, by not ensuring staff sign and date when medication is dispensed, and ensuring all medication is properly listed or removed if discontinued which poses an immediate health, safety or personal rights risk to persons in care.
POC Due Date: 04/28/2025 Plan of Correction The Administrator agrees to complete a staff training regarding the importance of logging medication, signing and date the medication was dispensed and will review the regulations and provide LPA with a sign in sheet of all saff who completed the training.
