(B) Bed rails that extend the entire length of the bed are prohibited except for residents who are currently receiving hospice care... This requirement is not met as evidenced by: Based on observations, interviews and records review, the Licensee did not comply with section cited above by allowing R2 to have full bed rail without Exception Letter approved by CCLD or R2 not on hospice care which poses potential health, safety and personal rights risks to resident in care.

This requirement is not met as evidenced by: LPA observation of full length bed rails on R1's bed who is not receiving hospice services. Deficient Practice Statement Based on review of R1's record, the licensee did not comply with the section cited above in one (1) out of three (3) residents with beds with full bed rails. This poses an immediate health, safety to persons in care.
