Transferability of License-(b)..., the property and business shall not be transferred until the buyer qualifies for a license or provisional license... This requiremnt was not met based on interviews and records. This poses an immediate risk to the residents.

Additional Personal Rights of Residents in Privately Operated Facilities- (16) To written notice of any room changes at least 30 days in advance unless a room change is agreed to by the resident, required to fill a vacant bed, or necessary due to an emergency. (a) Each residential care facility for the elderly licensed under this chapter shall ensure that each employee of the facility who assists residents with the self-administration of medications meets all of the following training requirements: (2) In facilities licensed to provide care for 15 or fewer persons, the employee shall complete 10 hours of initial training.
