What the official deficiency says

87615 Prohibited Health Conditions (a) Persons who require health services for or have a health condition including, but not limited to, those specified below shall not be admitted or retained in a residential care facility for the elderly: (1) Stage 3 and 4 pressure injuries. (2) Gastrostomy tubes. This requirement was not met based on interview and record that found R1 was admitted 6/17/24 with a g-tube. and R2 has greatwer than stage II pressure injuries. This posed and immediate risk to R1's health.

Additional Personal Rights of Residents in Privately Operated Facilities (a):(8) To be free from neglect, financial exploitation, involuntary seclusion, punishment, humiliation, intimidation, and verbal, mental, physical, or sexual abuse. This requirement is not met as evidenced by: Based on documentation reviewed, the Licensee did not ensure that resident (R1) was free from financial exploitation, while living at the facility, from on/around August 2022 through March 2023, before relocating to the licensee’s other licensed care home, which posed an immediate health and safety risk to residents in care.

Incidental Medical and Dental Care(h)(2) Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement was not met based on observation that medications were not safely stored or disposed of when expired. This posed an immediate risk to residents.
