What the official deficiency says

Incidental Medical and Dental Care Services ... 9-1-1 shall be telephoned immediately if an injury or other circumstance has resulted in an imminent threat to a resident's health including an apparent life-threatening medical crisis. This requirement is not met as evidenced by: R1 had an unwitnessed fall in which medical attention was delayed for approximately 4 hours after which it was later discovered R1 suffered multiple rib fractures. This poses an immediate risk to the health and safety of residents in care.

Reappraisals ...The reappraisal shall document significant changes in the resident's physical, mental, cognitive, behavioral, or functional condition, including those required to be documented as specified in Section 87466, Observation of the Resident … Significant changes in condition, as defined in Section 87101. This requirement is not met as evidenced by: Based on record review and interviews, the facility failed to ensure R1 received a reappraisal after significant changes in condition, including falls, hospitalization, increased supervision needs, and decline in functioning resulting in multiple rib fractures. This poses an immediate health and safety risk to residents in care.

What the official deficiency says

(h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. This requirement is not met as evidenced by: Based on LPA's observations the facility staff did not ensure medications were not transferred between containers which pose potential health, safety and personal rights risk to clients in care.
