What the official deficiency says
(c) If the resident's physician has stated in writing that the resident is unable to determine his/her own need for nonprescription PRN medication, but can communicate his/her symptoms clearly, facility staff designated by the licensee shall be permitted to assist the resident with self-administration, provided all of the following requirements are met: (2) Once ordered by the physician the medication is given according to the physician's directions. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and record review, Resident #4 did not take Omeprazole 40mg on 11/30/25 which poses an immediate health, safety or personal rights risk to persons in care. The pill remained in the bubble pack of annual visit on 12/01/25.

POC Due Date: 12/02/2025 Plan of Correction Licensee will ensure all residents take their medications as prescribed on a daily basis. All staff will be trained on this and proof will be sent to CCLD by POC due date.

(e) Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did maintain a water temperature between 105 to 120 degrees Fahrenheit which poses a potential health, safety or personal rights risk to persons in care. During the visit, the water temperature was between 129.2 and 130.6 degrees Fahrenheit.

POC Due Date: 01/01/2026 Plan of Correction Licensee will adjust the water heater, allow it to drain, and send proof to CCLD that the water is between 105 and 120 degrees Fahrenheit and send it to CCLD by POC due date.

What the official deficiency says
(a) In order accept or retain terminally ill residents and permit them to receive care from a hospice agency, the licensee shall have obtained a facility hospice care waiver from the Department. To obtain this waiver the licensee shall submit a written request for a waiver to the Department on behalf of any residents who may request retention, and any future residents who may request acceptance, along with the provision of hospice services in the facility. The request shall include, but not be limited to the following: (1) Specification of the maximum number of terminally ill residents which the facility wants to have at any one time. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, five residents are on hospice, while the facility has a hospice waiver for four, which poses a potential health, safety or personal rights risk to persons in care.
