(h) The licensee shall provide sufficient space to accommodate both indoor and outdoor activities. Activities shall be encouraged by provision of: (2) Outdoor activity areas that are easily accessible to residents, protected from traffic, and have adequate shady areas. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not have a shaded outdoor patio area which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 05/01/2026 Plan of Correction Licensee will obtain patio umbrellas for the outdoor furniture and provide proof to CCLD by POC due date.
(a) The pre-admission appraisal, as specified in Section 87457, Pre-Admission Appraisal, shall be updated, in writing as frequently as necessary or once every 12 months, whichever occurs first, to note significant changes in condition, as defined in Section 87101, Definitions, and to keep the appraisal accurate. For the purposes of this section, the updated pre-admission appraisal shall be referred to as the reappraisal. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and record review, the licensee did not have appraisals for Residents #3, and #5 which poses a potential health, safety or personal rights risk to persons in care.
POC Due Date: 05/08/2026 Plan of Correction Licensee will obtain resident appraisals for Residents #3, and #5 and submit them to CCLD via email or fax. Licensee will keep updated appraisals in the resident files at all times.
(e) Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degrees C) and not more than 120 degree F (49 degrees C). This requirement is not met as evidenced by: Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. The hot water in resident bathrooms measured 125.7 to 128.4 degrees Fahrenheit.
POC Due Date: 05/12/2025 Plan of Correction AD agreed to start documenting a hot water log of resident bathroom temperatures. LPA will conduct a follow up visit to ensure the hot water is at an appropriate temperature.
(a) Prior to accepting a resident for care and in order to evaluate his/her suitability, the facility shall, as specified in this article 8: (2) Perform a pre-admission appraisal.
