What the official deficiency says

(a) Prior to a person's acceptance as a resident, the licensee shall obtain documentation of a medical assessment, signed by a licensed medical professional .., to be kept in the resident's record. This requirement is not met as evidenced by: Based on interview and record review, no medical assessment, tuberculosis test, admission agreement, and required documents were completed for one of five residents. which poses a potential health, safety or personal rights risk to persons in care.

What the official deficiency says

(a) Prior to a person's acceptance as a resident, the licensee shall obtain documentation of a medical assessment, signed by a licensed medical professional acting within the scope of their practice and made within the last year, to be kept in the resident's record. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed that there was no Medical Assessment on file for Resident #5 (R5).

POC Due Date: 01/19/2026 Plan of Correction The House Manager stated that she will obtain a Medical Assessment for R5. The House Manager agreed to provide the Medical Assessment for R5 to LPA via email or fax by POC date.

What the official deficiency says

(h) The following requirements shall apply to medications which are centrally stored: (1) Medications shall be centrally stored under the following circumstances: (C) Because of potential dangers related to the medication itself, or due to physical arrangements in the facility and the condition or the habits of other persons in the facility, the medications are determined by either a physician, the administrator, or Department to be a safety hazard to others. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and records review, the facility did not comply with the section cited above during the medications review, one out of five of the medications of R4, for medication named PreserVision, did not have a doctor's order on file. This could pose a potential health risk to persons in care if medications are not determine/ prescribed by a physician.

POC Due Date: 01/07/2025 Plan of Correction During the visit, the Administrator(AD) was able to get a copy of the prescription from the family.
