87458 Medical Assessment (a) Prior to a person's acceptance as a resident, the licensee shall obtain documentation of a medical assessment, signed... and made within the last year. This requirement is not met as evidenced by: Deficient Practice Statement Based on observation of record review, LPA observed that the LIC602 for all 4 current residents are missing. Per interview with AD, AD confirmed that all 4 residents completed a medical assessment, but provided admission that LIC602s were not completed. Therefore, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above and did not lock disinfectants, cleaning solutions, poisons, and other items which could post a danger. LPA observed Lysol bleach spray under the bathroom sink and under the kitchen sink. This poses an immediate health, safety or personal rights risk to persons in care.
