What the official deficiency says
(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by: Deficient Practice Statement Based on documents and observation, the licensee did not ensure knives were inaccessible to multiple residents not assessed to be able to safety handle knives where the knives were in the child-proofed but non-lockable under sink storage, which poses an immediate safety risk to persons in care.

POC Due Date: 10/10/2025 Plan of Correction During the inspection, the licensee secured these items and LPA confirmed. Licensee stated they will retrain staff on securing dangerous items with real locks and submit proof to LPA by POC due date.
