(a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department or district providing fire protection services, or the State Fire Marshal. Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department or district providing fire protection services, or the State Fire Marshal: This requirement is not met as evidenced by: Deficient Practice Statement During annual inspection LPA observed expired fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. (1) Disinfectants, cleaning solutions, and poisonous substances shall be stored in areas separate from food supplies as specified in Section 87555, General Food Service Requirements. This requirement is not met as evidenced by: Deficient Practice Statement During annual inspection visit LPA observed cleaning solution (laundry detergent) not locked, which poses/posed a potential health, safety or personal rights risk to persons in care.

POC Due Date: 08/19/2025 Plan of Correction Administrator will designate proper area to store the cleaning solutions and will provide images to LPA by email by POC due date.

This requirement is not met as evidenced by: Deficient Practice Statement During annual inspection and records review LPA observed residents records missing recent medical evaluation which poses/posed a potential health, safety or personal rights risk to persons in care.
